- To complete the Supplier Business Information
Form, you have two options:
- Download an Adobe Acrobat® version of the Supplier Business Information form
- Link to UC Office of the President's Business Information site to download a MS Word version of the form to your own system.
- Fill out all the fields, including your
ownership status category on the second page. Definitions of the categories are available online.
- Sign and mail to UCSD at:

- If you are a Construction Contractor or Design Professional, please refer to the Maximum Receipts Table to help determine your eligibility to use the "Small Business" designation.
